As you head east along Interstate 64 in Kentucky, slowly but surely you go from the bluegrass to Appalachia. As you approach the mountains on the other side of Lexington, you enter some of the poorest parts of modern America. But there, tucked away in rolling hills and mountains that run from Pennsylvania to Georgia, you find an enigma.
