Susan (Carole Ann Ford) is menaced by a Voord in The Sea of Death, the opening episode of The Keys of Marinus (1964). This was the first episode that Graham Strong recorded.
It’s a tragedy that 97 episodes of Doctor Who are still missing from the BBC archive, but we can take consolation from the fact that a handful of dedicated fans made it part of their weekly ritual to record the soundtracks of episodes on reel-to-reel tape machines. It’s thanks to people such as the late Graham Strong that we have a complete audio record of every episode of Doctor Who ever transmitted.
