There are a couple of very capable runners-up here.
ArtRage Vitae has a great feature set and is fun to use but its interface could use a little sprucing up. ArtStudio Pro is also powerful enough for most users and adds a degree of photo-editing support into its modern if slightly clinical interface. iPastels is fun but suffers from a basic feel, despite having a solid toolset. Clip Studio Pro is almost in a different category given its focus on heavyweight design tasks and, despite its art capabilities, would probably be overkill for many users.
