THE Rail Accident Investigation Branch (RAIB) – the body, which independently investigates accidents to improve railway safety and inform the industry and the public alike – has issued what it calls ‘urgent safety advice’ regarding passenger doors which are not centrally controlled or locked by traincrew.
RAIB said it issues such notices when it wishes to quickly communicate important safety learning and/or when it has identified the need for a particular risk to be urgently addressed.
